An NDIS plan is only useful if you can actually use it
Getting an NDIS plan is one thing. Turning that plan into supports that actually change your week is another. The system has its own language, its own timelines, and its own ways of going wrong. For young people leaving school, for families starting out, or for anyone whose situation has just changed, the gap between funding on paper and help in real life can feel enormous.
Support Coordination through DAYS closes that gap. DAYS (Disability and Youth Support) is our sister organisation under DAY Group Management. It works alongside GCDAY across the Gold Coast and South Brisbane, with its own coordinators and SLES team. We work alongside you to understand what you actually want out of your plan, then connect you to the right supports to get there. That might mean finding reliable providers, setting up services, managing your budget, or adjusting things when life shifts. We don't just refer you on and disappear. We stay involved, check in, and step in when things aren't working.
What's involved

A team that knows the system from the inside
Our coordinators come from a mix of professional backgrounds in counselling and social work, with ground-level experience as support workers. We bring lived experience to the role too, so we understand the realities of what we're coordinating, not just the policy around it. Between us, we've worked through NDIS reviews, AAT processes, and Centrelink overlaps enough times that the federal-level mess doesn't faze us. We're neuro-spicy affirming and LGBTIQ+ supportive, and our work is built on creating a space where you feel safe to be honest about what you actually need.

Practical, not paper-based
Support Coordination is hands-on work. Clear guidance, honest conversations, and someone in your corner making sure your plan is doing what it's meant to do. We help you compare providers, untangle Centrelink and NDIS overlaps, and adjust your supports when something stops working. The goal is to leave you more in control of your own plan, not more dependent on someone else to manage it.

School Leaver Employment Supports
For young people stepping out of school, DAYS also runs a dedicated program focused on the move into work and adult life. Through practical activities, real-world experiences, and one-to-one support, we help build job readiness, routines, communication skills, and the confidence to engage with the community. The pathway is shaped around each participant's strengths and interests, with the long-term goal of meaningful employment and ongoing participation.
